When you begin your dental experience with us, you will receive a Patient Confidentiality Code and email so that you can submit all of your information. We need the following information to begin your dental treatment, and to give you the most accurate price quote possible.

- - Prescriptions or medications
- - Scanned dental documents
- - Describe any medical issues and your dental history
- - Scanned x-rays from your dentist office
- - CT Scan
- - Pictures of your smile, lower arch, and upper arch. A digital camera or smart phone will usually suffice.
Information Required:
- Let us know any kind of Medical Issues and prescriptions tour taking, please describe them and rake some pictures of the drugs your taking
- Explain with detail your dental history, and the most important are you expecting of the Dental Treatment.
- Take some picures of your smile and your actual dental condition (you can ask someone with a digital camera or cell phone, please wit e ough light and flash).
- If you have any Dental papers please summit them to us (you can take them a clear picture or scan them)
- Send us xrays (if you have), just scan them, for a clear scan we recommend to do it in a clear room, with lots of light and the top of the scanner open.
- If you have a CT SCAN , just ZIP it and send it to our email
